DHMH Survey for Maryland Parents of Children with Disabilites or Special Health Care Needs

Every five years the Department of Health and Mental Hygiene must conduct a Needs Assessment to determine where money will be spent and the need for more or different programs to be available for families of children with special health care needs and disabilities.
This is your opportunity to make a difference! The Parents' Place of Maryland is asking you to spend about 10 minutes filling out the survey found at the link provided below.
This survey is completely anonymous and will be used for the purpose of collecting data on the needs of families of children across Maryland. DHMH contracted with the Parents' Place to collect information directly from families. The survey was designed and is distributed by Maryland families in partnership with providers across the state.
We truly appreciate the time you spend and the information you share that will have a direct impact on services and supports for children and families in this state.
